– ***rmation on previous therapy,including responses or side-effectsa treatment could be mistakenly recordedas ineffective when the real problemwas under-treatment or incorrectuse of the prescribed treatment, ordiscontinued as unsuitable whentransient side-effects could have beenovercome had more advice been given
– any relevant background ***rmation,including the patient's generalhealth and current medication
– the patient's home circumstances;important because the patient'sability to apply topical therapies ataffected sites may be compromised,affecting treatment choice
